What is KidCare? 

It's a state program that offers health care coverage to children, pregnant women and their babies or helps in paying premiums of employer or private health insurance plans

School Medication Authorization Form (pdf format)
         To be filled out if student will need medication administered
         during the school day

IDPH Certificate of Child Health Examination (pdf format)- All freshmen must have an IDPH Certificate OF Child Health Examination form completed by a physician prior to entering 9th grade. Any student transferring from another school will have to have the Health Examination form completed if a current physical is not on file at previous school.
