What is KidCare? 

It's a state program that offers health care coverage to children, pregnant women and their babies or helps in paying premiums of employer or private health insurance plans

School Medication Authorization Form - To be filled out if student will need medication administered during the school day

IDPH Certificate of Child Health Examination - All freshmen must have an IDPH Certificate of Child Health Examination form completed by a physician prior to entering 9th grade. Any student transferring from another school will have to have the Health Examination form completed if a current physical is not on file at previous school.
